In order to register for one of our summer camps, you must print out a copy of the health form and registration form, fill them out, and mail them in with your deposit. The health form should be submitted with the registration form.
IMPORTANT!!
All campers MUST complete a health form, which can be downloaded below. This form should be mailed in with the registration form. Mail to:

No camper will be allowed to attend camp without a health form on file.
You can download our forms below. For either of them you may either (1) click on the link to open the file in the browser, then print, or (2) right click on the link (control-click for Mac users), and choose "Save link as..." or "Save target as..."; then navigate to the location of the hard drive or floppy disk where you would like to download the file; you may then open the file with other software to print when convenient.
